Church Facilities Utilization Application Guidances 教會場地及設施使用申請指南

To make the best use of the church’s facilities for everyone, brothers and sisters are encouraged to consider the needs of other ministries in addition to their own (Philippians 2:4). Please choose a room whose capacity and facilities are appropriate for your event. In some cases, multiple users of the same room may need to discuss and agree on a shared arrangement.
1. Booking Responsibility
The person submitting the booking form is considered the Event Coordinator, who will oversee room setup, cleanliness, and proper use of equipment during the event.
Each Ministry or Fellowship Coordinator serves as the Approver for their group’s bookings. All room bookings must be approved by the respective coordinator before they can be confirmed.
2. Booking Procedures
Please submit your booking at least one week before the event, unless in exceptional circumstances.
Before submitting, check the 675/685 Facility Utilization Calendar to confirm availability.
If the room is already booked, please consider another time or room. If your ministry must use the same room, contact the Church Office, which can connect you with the other group to discuss a suitable arrangement.
The Church Office will notify you of your booking status after approval.
If you no longer need the room, please cancel your booking promptly by informing the Church Office.
3. Event Coordinator’s Responsibilities
Every event must have an Event Coordinator responsible for:
Setting up and restoring the room to its original condition.
Turning off lights, fans, and equipment after use.
Ensuring compliance with safety and operational guidelines.
Returning any borrowed items (e.g., handcart from Room 307 storage at 685).
Contacting:
Peter Ng (416-918-6223) for help with room equipment.
Leo Wu (416-417-4198) for locked rooms (available from 7:00 pm Mon–Fri, 6:00 pm Sat; details posted on each floor).
4. Gym Booking for Sports Activities
All participants must complete a Release Waiver Form the first time they join a specific sporting event.
If a participant joins a different sporting activity, a new form must be submitted.
5. Parking Arrangements
The coordinator should remind fellowship participants to apply for an E-Parking Permit on the church website.
For non-fellowship events (e.g., sports only), participants should apply for a paper parking permit at least two days in advance.
The coordinator may request a one-day parking permit (outside office hours) in advance if needed — please contact the Church Office.

各事工或團契的 負責人(Coordinator) 為該組別的 審批人。所有房間預訂必須經由負責人批准後方可確認。
2. 預訂程序
除特殊情況外,請最少於活動前 一週 提交預訂申請。
提交前,請先查閱675/685 設施使用日曆,確認場地是否可用。
若場地已被預訂,請考慮使用其他時間或房間。若您的事工必須於該時段使用該場地,請聯絡教會辦公室協助與對方協調安排。
若場地可用,請填寫 線上設施使用申請表。教會辦公室會在負責人批准後通知您申請結果。
若活動取消或不再需要使用場地,請儘快通知教會辦公室取消預訂。
3. 活動負責人責任
每項活動必須有一位 活動負責人,負責:
活動前後整理及恢復場地原狀。
關閉所有燈光、風扇及設備。
確保活動期間遵守安全及操作守則。
借用物品(如685大樓307房儲物室手推車)後須歸還原位。

4. 體育館使用規定
所有首次參加某項體育活動的參與者,必須填寫《免責聲明表》。
若參加另一項體育活動,需重新填寫新表格。
5. 泊車安排
負責人應提醒參加團契的弟兄姊妹於教會網站申請 電子泊車證。
若參加者僅出席體育活動(非團契),應最少提前兩天申請 紙本泊車證。
負責人如需於非辦公時間預訂 一日泊車證,可提前聯絡教會辦公室申請。
